Office of Institutional Effectiveness, Planning and Research (OIEPR) helps conduct the student evaluation of course survey toward the end of the Quarter. Please click here to download the Student Evaluation Form available in PDF format and distribute to the students in class. Please follow the below mentioned instructions carefully for a successful processing:

* Please PRINT as many copies as you need for the students. DO NOT attempt to make photocopies of the form as the scanning software does not recognize the photocopied forms. If you have difficulties printing the form, OIEPR will be glad to print the forms for you. Let us know in advance.

* Each form needs to be printed on a single page by printing on both the sides of the page front-toback. Please check your printer setting before printing the form and if you have difficulties, let us know.

* Distribute this form for each course that is being evaluated. For unbiased response make sure the faculty member teaching the course is not present in the classroom while students are responding.

* Collect the forms from students and place them all in an envelope along with the cover sheet to be delivered to OIEPR located in Mod 1 on campus. If you have any questions, please call our office at 770-426-2654 or by email institutional.research@life.edu.
