Verification Forms

After FAFSA information is received, some students are chosen for a process called Verification.  If you are required, the following items are available in PDF format. 

DO NOT COMPLETE THESE FORMS UNLESS REQUESTED BY THE FINANCIAL AID OFFICE

Once printed and completed the forms may be mailed, faxed, or delivered to the Office of Financial Aid.

Please make sure your forms are fully completed and signed.  Failure to complete and/or sign forms will significantly delay processing.
