The Financial Aid forms below are either submitted electronically through DocuSign OR emailed to FinAidMail@life.edu. This address is for forms only and will not accommodate replies.
Verification of Financial Aid for 3rd Parties This form is for students needing an apartment letter or verification of financial aid funds for 3rd parties.
Budget Increase Policy and Request | This form is for students wanting to request an increase to their cost of attendance for emergency expenses incurred out-of-pocket. Requests may include review for emergency health/dental care, psychological testing and disability accommodations, dependent care expenses, national chiropractic board exam costs and travel expenses, or a one-time computer purchase. The Budget Increase Reference Sheet details what you will need to submit for each category.
Consortium Agreement Application | This form is for students interested in registering at another college/university for financial aid purposes.
Establishing Georgia Residency Form 2024-2025 | Establishing Georgia Residency form 2025-2026 AND HOPE Evaluation Form| These two forms are for students with Georgia residency who want to apply for the HOPE Scholarship and/or the Georgia Tuition Equalization Grant.
Financial Aid Loan Adjustment Form | This form is for students wanting a re-evaluation of eligibility or to change loan amounts.
Special Circumstances Appeal/PJ Form | This form is for students to request consideration of a special/unusual circumstance to be reviewed by our office.
Total & Permanent Disability Discharge Form | This form is for students to reestablish eligibility for Federal Student Loan Programs when prior loans have been discharged due to total and permanent disability.
Tuition Benefit – Application to Disburse Benefit | This form is for students wanting to apply to receive the tuition benefit if married or family member. See form below for application to apply for benefit.
Tuition Benefit – Married & Family Member Application | This form is for students wanting to apply to receive the tuition benefit if married or family member. Click here to view the Tuition Benefit Policy in full.
Tuition Benefit – Termination of Benefit Form | This form is for students needing to notify Life of termination of qualifying relationship for the tuition benefit.
Work Study Adjustment Form | This form is for students wanting to increase, reduce, or cancel awarded Work Study funds.
